REQUESTING AN APPOINTMENT
Please call our office at (818) 500-8739 or Request an Appointment online. When you call or Request an Appointment, we will verify your health insurance coverage and ask you to pick up a laboratory request form for a blood test. If you are unable to visit our office to pick up the form, we will provide you with special instructions when you call us. We will also advise your appointment confirmation with Dr. Protacio at that time.
We will wait for your blood test results for about a week or two in preparation for your appointment with Dr. Protacio.
WHAT TO BRING TO YOUR APPOINTMENT
Immunization card if the appointment is for a well-child exam/physical.
Proof of insurance if the patient is a minor or below 18 years of age or if your plan to pay for a non-work related injury or illness with insurance (*Please note that we only see patients starting from 12 years old and above).
Proof of guardianship or notarized letter from parents or guardians authorizing treatment to a minor if the minor is accompanied by an adult who is neither the minor’s parent or guardian and valid form of identification
Additional Information (RECOMMENDED)
It is also recommended that you are ready with the following information on your appointment:
Medical history information - past surgeries, medical conditions and allergies
A list of the patient’s current prescriptions and over-the-counter medications.
Forms you need your clinician to fill out, e.g. school physical forms.
Any insurance copay is due at the time of your visit. We accept cash or personal checks. At this time, we are unable to accept debit or credit cards.
Most insurances are accepted
Validated parking for patients within the building vicinity is available during business hours.

If you are looking to be referred to a specialist, you would need to become a patient first and have your first appointment with us. Please call Dr. Protacio’s office at (818) 500-8739 for instructions or for any other inquiries even if you are still not our patient.
NOTICE OF PRIVACY PRACTICES
Under the Health Insurance Portability and Accountability Act of 1996 (HIPPA), patients have certain rights to privacy regarding their protected health information. At California Primary Health Care, we take security measures to keep your information safe. Information will be used to:
1) Conduct plan and direct patients' treatment and follow-up among the multiple health care providers who may be involved in patients' treatment directly and indirectly.
2) Obtain payment from third party payers
3) Conduct normal healthcare operations such as quality assessments and physician certifications.
California Primary Health Care may change this Notice of Privacy Practices from time to time. You may contact us anytime to obtain a current copy of the Notice of Privacy Practices. When you become our patient, we also ask you to acknowledge our Notice of Privacy Practices through an acknowledgment and consent form authorizing disclosure of protected health information to certain individuals you choose to receive your information. The authorization of disclosure may be revoked by a written notification and is valid until such written request is submitted.
