Welcome to our Patient Portal!

 

We encourage all our patients to enroll in our patient portal. We have partnered with NEXTGEN to communicate with our patients in a convenient, safe and secure environment. Patients who are enrolled to our Patient Portal are able to:​

  • View their charts with health record details 

  • Send or receive secure messages from our providers or request personal health records (PHRs) such as lab test results

  • Receive and review other important patient documents

  • Request medication renewals

 

If you have already been enrolled to our Patient Portal, please access log in below or visit the NEXTGEN Patient Portal site by clicking here.

 

If you are an existing patient at California Primary Health Care and have not been enrolled, please call our office at (818) 500-8739 to be enrolled and receive further instructions via email.  

  1. When you call us, we will ask you to provide us with personal details such as your name, address, phone number and an email address. 

  2. For you to be enrolled, you will need an eight-digit token number from us which we will provide via email confirmation after you've called us for your enrollment.  

  3. You will use your token to complete your enrollment in the option below (click on I AM NEW HERE). Once you complete the enrollment process, you will no longer need your token as you will be asked to set-up your account with a new username and password.  

  4. You must complete the online enrollment process within 30 days after which the token will expire and you will have to request from us a new token number.
